SOCIAL WORKER
The Social Worker collaborates with the patients to address challenges through a process of assessment, diagnosis, treatment, and evaluation in order to achieve optimum psychosocial and social functioning. To do this, the Social Worker relies heavily on the knowledge of and access to their community resources. This is often achieved by the means of collaborative work, continuing education, networking, and communication.

REGISTERED NURSE & REGISTERED PRACTICAL NURSE
The Registered Nurse and Registered Practical Nurse both provide high level of comprehensive care to meet the physical, emotional, social and spiritual needs of the individual, while participating in a team-oriented approach to patient care within the facility and in the promotion of continuing, family-centered care in the community.

NURSE PRACTITIONER
The Nurse Practitioner provides a full range of health care services to individuals, families, and communities. Nurse Practitioners diagnose and treat illnesses and/or injuries, orders and interprets diagnostic tests, writes prescriptions, provides treatments and/or procedures and makes referrals to family physicians, specialists and other health professionals.

PHYSIOTHERAPIST & KINESIOLOGIST
The Registered Physiotherapist and Kinesiologist is responsible for assessment, treatment planning, treatment provision-manual therapy, exercise, education, and diagnose and treat symptoms of illness, injury or disability. Services range from pediatrics to senior.
ROLE OF CLINICAL PROVIDERS
Our family physicians work collaboratively with other health care professionals on our team to provide you with comprehensive family medicine services.
