The Community Care Clinic provides primary health care services for individuals who do not currently have a primary care provider.

Our clinic offers comprehensive care through a Nurse Practitioner and nursing staff, including assessment, treatment, health education, chronic disease support, preventive care, and referrals to additional services when needed.
